This program provides personalised, culturally safe care designed to help you age well. From comprehensive health checks to coordinated support and allied health services, the EIHA program helps you stay healthy and connected to the care you need in North Western Melbourne.


Personalised Care for Ageing Australians
Our program is designed for individuals at risk of chronic health conditions, specifically:
- Adults aged 50 to 74 years old.
- Aboriginal and Torres Strait Islander people aged 35+.
- Culturally and Linguistically Diverse (CALD) individuals.
- LGBTQIA+ individuals.
Why Prioritise Early Intervention?
As we age, managing long-term health conditions becomes a priority. Nearly nine in ten older Australians manage at least one ongoing health issue.
Joining the EIHA program helps you:

What EIHA Offers
Supported by funding from the North Western Melbourne Primary Health Network, most of our services are bulk-billed (please note GP appointments are privately billed). Our services include:
- Assessments focused on What Matters, Medication, Mentation and Mobility.
- Custom health roadmaps with regular reviews.
- Direct paths to physiotherapy, dietetics, pharmacy, and mental health support.
- Monthly follow-ups and multidisciplinary case conferences.
- Support for self-management and social connection.
Referral Form
Personal Care & Companionship
Our carers can help you prepare and cook meals, or arrange for them to be delivered from a provider like Lite n’ Easy. Your Care Plan can cover the cost of preparing and transporting meals.
Domestic Assistance & Transportation
We very much value helping an individual stay connected. We can help you with transportation, attend social outing, family gatherings, recreational and volunteer activities.
Nursing
Our enrolled and registered nurses can assist you with your medical needs. This might include providing wound dressings, dispensing medications and administering injections.
Allied Health
Our expert Podiatrists, Physiotherapists, Occupational Therapists and Audiologists work together to keep you mobile, independent and pain-free. From foot care and injury management to mobility support and hearing aid fittings, we provide holistic care tailored to your needs.
Telehealth and Remote Patient Monitoring
There are many ways that technology can assist and improve care at home. Many of these are now rapidly advancing. Finding which technological aid is best suited for you, and indeed, when and how to use them, can be tricky.
Home Maintenance
We have gardeners who can mow your lawns and tidy up your garden. For larger scale jobs involving big trees, we can engage our trusted contractors.
